1. Model Name – Example: Littmann Classic III, Cardiology IV, etc.
2. Serial Number – Engraved on the chestpiece or printed on the packaging.
3. Date of Purchase – You can find this on your order invoice.
4. Place of Purchase – Select or enter: stethoscope.com.ph
5. Your Contact Details – Name, email address, and phone number.

Where to Find the Serial Number?

• Engraved on the metal chestpiece of the stethoscope.
• May also appear on the product label on the original box.
